Your Financial Statement is ready. What to do next

This is a free tool. We are not lawyers and this is not legal advice. You fill in and sign the form yourself.
  1. Check and sign

Open the PDF and read both pages. If something is wrong, fix it here and download again. Sign page 2 by hand under penalty of perjury. If you checked “My spouse has signed below,” your spouse signs too.

  2. Attach it to your claim of exemption

It goes with WG-006 (wage garnishment, items 3b or 3c) or EJ-160 (bank levy, item 4). Attach any page labeled Attachment 6 if you need more space.

  3. Make copies

The court’s guides say: make two copies of your completed forms and keep one for yourself.

  4. Give both forms to the levying officer, not the court

Take or mail the original and one copy to the levying officer named on your Earnings Withholding Order (WG-002) or Notice of Levy, usually the sheriff. The court’s form page says the Financial Statement tells the levying officer and the other side about your finances, and if your claim is opposed this information is forwarded to the court. 5. If there is a hearing

The guides say to bring evidence such as paychecks, bank statements and bills to show you can’t afford to have the money taken.

Good to know

  • This form shows your income, property and debts. Our tool makes the PDF on your device and does not keep your answers. The form itself suggests pressing its “Clear This Form” button after printing if you fill it out on a shared computer.
